What this screener does

This is a short, free symptom check, not a diagnosis. It captures four data points: how burnout is showing up day to day, what physical symptoms you have, what kind of job and benefits situation you are in, and what you are most worried about if you take time off. Those four answers go to our Clinical Intake Director, who uses them to prepare a more efficient first phone call.

Who it is for

Working adults in the United States dealing with occupational burnout. Especially relevant for NYC professionals: tech founders, finance, healthcare workers, creative roles, and anyone whose nervous system is showing the cost of staying in their job too long.

What happens next

If you live in New York, Florida, or New Jersey, our Intake Director calls or texts within 1 business day. If you are outside those states, you go on a waitlist and, with your permission, we route your information to a vetted local clinician who specializes in burnout and medical leave.

Your privacy

Your screener answers stay with our clinical team. PIM never contacts your employer without your written consent. When we file claims for paid medical leave, we deal directly with the third-party insurance carrier (Sedgwick, Lincoln Financial, Guardian, MetLife). Your HR team typically only sees a generic notice that you are on approved leave.

Frequently asked questions about burnout and medical leave

Can I take medical leave for burnout in the United States?

Burnout is not a stand-alone diagnosis in the US DSM-5. The World Health Organization classifies it as an occupational phenomenon in ICD-11. However, the symptoms of burnout, including sleep disruption, anxiety, cognitive impairment, and depression, often meet criteria for diagnoses like Adjustment Disorder, Major Depressive Episode, or Generalized Anxiety. Those diagnoses qualify for medical leave under FMLA, short-term disability policies, and state programs.

Will my employer know my diagnosis?

No. Under HIPAA, your medical records are private. When PIM submits paperwork for short-term disability or FMLA leave, we communicate directly with the third-party insurance carrier (Sedgwick, Lincoln Financial, Guardian, MetLife), not your employer. Your HR department typically receives only a generic notice that you are on approved medical leave with an expected return date.

Can I be fired for taking medical leave?

Retaliation for taking FMLA-protected leave is illegal under federal law. The Family and Medical Leave Act protects your job (or an equivalent one) for up to 12 weeks of qualifying unpaid leave. The Americans with Disabilities Act adds further protections. Firing or demoting someone on documented medical leave creates significant legal liability for the employer.

How much of my paycheck do I get during medical leave?

It depends on which protection you use. FMLA itself is unpaid. New York Disability Benefits Law caps weekly benefits at 170 USD. Private short-term disability (the policy your employer pays for through providers like Lincoln Financial or Guardian) typically replaces 60 to 100 percent of your salary for the duration of leave. The Private STD path is what PIM specializes in helping patients document and access.

What states does Psychiatry in Motion serve?

PIM provides direct psychiatric care to patients in New York, Florida, and New Jersey, with telehealth available in NY and FL. If you live in another state, you can still take the screener. Out-of-state respondents are added to the practice waitlist and, with their permission, connected to a vetted local clinician who specializes in burnout and medical leave.

Is burnout actually a real medical condition?

The World Health Organization recognizes burnout in ICD-11 as an occupational phenomenon characterized by three dimensions: feelings of energy depletion or exhaustion, increased mental distance from one's job or cynicism, and reduced professional efficacy. The underlying symptoms (insomnia, anxiety, cognitive impairment, depressed mood) are clinically recognized and treatable, even when the label burnout itself is not yet a stand-alone diagnostic code in the DSM-5.

How long does the screener take?

About 90 seconds. Four short symptom and situation questions, then a brief contact form so the Intake Director can follow up within 1 business day.

Do I need insurance to take this screener or to work with PIM?

The screener is free regardless of insurance status. PIM is in-network with Aetna and UnitedHealthcare (including Oscar, Optum, and Oxford) in New York. Cigna and BCBS are processed through Headway. We provide automatic superbills for out-of-network reimbursement on cash-pay and concierge services. HSA and FSA funds are accepted for qualifying expenses.

A score from this screener is not a medical diagnosis. It is a data point that helps our clinical team prepare for your first conversation. If you are in a mental health crisis, please call or text 988 to reach the Suicide and Crisis Lifeline, or go to your nearest emergency room.

What this screener checks — and why it matters

Burnout is not just feeling tired. It's a recognized occupational phenomenon (ICD-11) with clinical symptoms — sleep disruption, anxiety, cognitive impairment, nervous system dysregulation — that often meet criteria for treatable diagnoses like Adjustment Disorder, Major Depressive Episode, or Generalized Anxiety.

Depending on your state, your employer's benefits, and how your symptoms are documented, you may qualify for paid medical leave under FMLA, short-term disability, or state programs like New York's Paid Family Leave.

A score from a screener is not a diagnosis. It's a data point that helps a real clinician prepare for your first conversation.

What happens with your answers

Your answers route directly to Psychiatry in Motion's clinical intake team — board-certified psychiatry serving New York, Florida, and New Jersey, with telehealth in NY and FL. If your situation fits, our Intake Director calls you within 1 business day.

If you're outside our service area, we add you to our waitlist and — with your permission — connect you with a vetted local clinician who specializes in burnout and medical leave.
