“But I Don’t Wash My Hands…”
OCD is a shapeshifter. It attacks what you value most. If you care about being a good person, it tells you you’re bad. If you care about your partner, it tells you to leave. Do any of these sound familiar?
Relationship (ROCD)
“Do I really love them? Are they ‘The One’?” You analyze their flaws, check your own feelings constantly, or confess doubts to feel relief. It looks like commitment issues, but it’s OCD.
Harm & Responsibility
“What if I snap and hurt someone?” “Did I hit a pedestrian with my car?” You are terrified of being secretly dangerous. You avoid knives, news stories, or driving to prevent a catastrophe only you can see.
“Just Right” & Order
It’s not about germs. It’s about a physical sensation of wrongness. If an email isn’t worded perfectly, or an object isn’t aligned, you feel a visceral tension (an “itch”) that you can’t ignore until you fix it.
Moral Scrupulosity
“Did I lie?” “Am I secretly a bad person?” You obsess over ethics, replaying conversations for days to check if you offended someone. You apologize excessively.
Somatic / Sensory
Hyper-awareness of blinking, breathing, or swallowing. Or intrusive “mental images” (flashes of violence/sex) that feel like hallucinations but aren’t. (Often misdiagnosed as psychosis in POC populations).
Existential OCD
“What is the point?” “Do I really exist?” You get stuck in endless philosophical loops that prevent you from living your actual life. It feels like “deep thinking,” but it’s actually a trap.
The “Itch” Analogy
Think of OCD like a mosquito bite on your brain.
“Did I lock the door?” “Do I love my partner?”
You check the lock. You ask your partner for reassurance.
The itch goes away for 5 minutes. But because you scratched it, the mosquito comes back bigger next time.
The only way to heal the bite is to stop scratching.
The ADHD vs. OCD Trap
You might think you have ADHD because you can’t focus. But why can’t you focus?
- ADHD is a deficit of regulation. You want to focus but get distracted by a bird.
- OCD is a surplus of anxiety. You are in an apathetic state of distraction because your brain is exhausted from ruminating. You are avoiding the work because the work triggers “Just Right” anxiety.
Treating OCD with ADHD stimulants often makes the anxiety worse. We must distinguish the two.

We use the “Medical Stabilizer” model. We don’t want you on high-dose SSRIs forever. We use them to lower the volume so you can do the work.
1. Medical Stabilization
We use medication (typically SSRIs at specific doses) to reduce the intensity of the “Itch.” This gives you the mental bandwidth to engage in therapy.
2. ERP Therapy (NOCD)
We refer out to NOCD for Exposure Response Prevention (ERP). This is the gold standard. We handle the meds; they handle the training. This combination is the cure.
3. The Taper
Once you have the skills to handle the “Itch” without scratching, we gladly take you off the medication. Our goal is your autonomy, not lifelong prescriptions.
